Genetic Testing or U.K. Population Surveillance*

The new Chief Executive of the National Health Service (NHS) in England, Simon Stevens, was recently reported arguing that the NHS must be transformed to make people’s personal genetic information the basis of their treatments.

z-dna_orbit_animated.gifHis proposition is unsurprising since it is in line with the efforts of successive UK governments to build a DNA database in the NHS in England by stealth. In particular, sequencing every baby at birth and storing whole genomes in electronic medical records is a plan backed by Health Secretary Jeremy Hunt. The current version of this plan would involve sharing whole or partial DNA sequences (genomes or genotypes) with companies like Google, which would use genetic information and health data to calculate personal risk assessments for feedback to patients. Massive investment from taxpayers would be required as part of a public-private partnership that would allow commercial exploitation of the data.

Building a DNA database within the NHS would be a massive waste of public money. But it would also create a system of total surveillance which would enable the government and private companies to track every individual, not to mention their relatives. This is not speculation; as wikileaks revealed, the United States government is already actively collecting DNA samples and biometric data on foreign officials and populations.

Commercial companies wish to exploit genetic information to market products such as drugs and supplements to healthy people, based on genetic risk assessments. If current trends continue, this will harm, not benefit, health: it is personalised marketing not personalised medicine. The potential for misuse is very high. Doctors could be replaced by computer algorithms used to market medication, massively expanding the drug market to include large numbers of healthy people, rather than smaller numbers of (often poorer) people who are sick. There is also the danger that prescribing would be driven by vested interests, rather than medical need: with high financial costs and more harmful side effects. Genetic risk assessments could also be misused, leading to stigma or discrimination, for example by insurers.

Why does the NHS want to collect genetic information?
There is one element of truth to Simon Stevens’ remarks. Some cancer drugs have been successfully tailored to genetic mutations that arise in the cancer tumour. However, attempts to select drugs for people based on the genetic make-up they are born with (their genome or genotype) have largely been a failure. This is because genetic differences only account for a part of individual differences in metabolism. For example, a recent study found that targeting warfarin treatment based on genetic make-up did not improve health outcomes, although this application was regarded as the ‘poster child’ of this approach

Why did it not work? An important misconception, apparently shared by Simon Stevens, is that genes are good predictors of most diseases and adverse drug reactions in most people. However, contrary to misleading claims made to promote the Human Genome Project, this is not true. Moreover, even if it were true, there is no evidence that genetic selection of individuals, for example into high risk and low risk groups, improves outcomes or cuts costs. All of which begs the question of the purpose of taking and storing genetic information as a default medical procedure.

The online gene testing company 23andMe, funded by Google, has been forced to withdraw its gene tests from the US market due to failure to prove they can reliably predict individual risks of many common conditions using computer algorithms. The company now wants to target the UK market, where genetic testing is not regulated. Patrick Chung, a 23andMe board member and partner at the venture-capital firm NEA told Fast Company:

“…23andMe will make money by partnering with countries that rely on a single-payer health system.

“Let’s say you genotype everyone in Canada or the United Kingdom or Abu Dhabi,” he says, “and the government is able to identify those segments of the population that are most at risk for heart disease or breast cancer or Parkinson’s. You can target them with preventative messages, make sure they’re examined more frequently, and in the end live healthier lives, and the government will save massive expenses because they halted someone who’s prediabetic from getting diabetes. 23andMe has been in discussion with a bunch of such societies“.

Yet there is not a scrap of evidence that this approach is good for health. This is because genomic tests have limited clinical validity or utility; so in reality there is no health benefit to targeting segments of the population in this way.

Genetic testing remains useful to diagnose rare genetic disorders, mainly in babies and young children, and whole genome sequencing has helped to identify new mutations causing these diseases. Rare familial (largely inherited) forms of many common diseases also exist, including breast cancer, but these account for only a small percentage of cases of these conditions.

Use of genetic testing in the NHS should focus on prioritising resources for the limited applications that do work, not on introducing misleading and harmful screening of the whole population and creating unnecessary, expensive databases. Certainly it should not be driven by ulterior commercial and government interests.

I have been traveling to East Asia (and many other parts of the world) for more than 25 years and over that time one of the things that has always struck me is how intelligent the general public in countries like Japan appear to be. It’s not that there aren’t dummies in East Asia, but it always seems that the average level of education and ability to think about the world intelligently and critically is impressively widespread. I’ve often thought about why this is the case and also why the same seems more difficult to say about the U.S. The answer, I think, can be found in a comment science fiction writer Isaac Asimov made about the U.S. while being interviewed in the 1980s:

“There is a cult of ignorance in the United States, and there has always been. The strain of anti-intellectualism has been a constant thread winding its way through our political and cultural life, nurtured by the false notion that democracy means that my ignorance is just as good as your knowledge.”

Asimov is right on the mark, and this cult of ignorance is the most serious national security issue facing the U.S. today. It is more important than the external threats from terrorists or the rise of a politically and economically powerful China. And a major part of the reason it is such an major issue for Americans to fix is that our immediate competitors, particularly those in Asia, have managed to create a culture in which rather than a cult of ignorance, a cult of intelligence plays a major role in shaping attitudes about the world and, thus, policies about dealing with other countries.

Many Americans are aware that the U.S. does not score well on measures such as international student assessment tests when compared to other industrial countries. For example, the 2011 Trends in International Mathematics and Science Study (TMISS) the top five countries for math were Singapore, South Korea, Hong Kong, Taiwan, and Japan—the U.S. is not in the top ten. It is better by 8th grade, where the same counties are in the top five (although the order changes) and the U.S. makes number 9. Roughly the same pattern can bee seen for science results. This doesn’t seem too bad, but in a different testing organization’s measure, the Programme for International Student Assessment, the U.S. does not fare quite so well, scoring 36th for math, 28th for science, and 24th for reading. With the exception of science, where Finland is ranked 5th, all of the top five countries in this measure are from East Asia.

American policy has generally worked from the assumption that the problem lies in basic weaknesses in the structure of our educational system with its inherent inequalities and the way in which our school curricula are constructed. These certainly have contributed to comparatively weak scores. I have long been convinced that one of the reasons Japan’s educational system is better than the U.S.—at least in the sense that a very broad swath of the general public receives a good and equal education through high school—is related to funding. The U.S. system generates inherent inequalities in school funding by depending upon property taxes. Even in states where there is some (usually grudging) redistribution of wealth to support public schools in poor areas (in Texas it is called the Robin Hood law), it is obvious that children in wealthy areas receive a better education with far greater academic and other resources than those in poorer areas. In Japan, because there is a national curriculum and a significant portion of the funding for public schools comes from the national government, in addition to funding from prefectural and municipal governments, there is considerably less inequality in distribution of and access to quality education than in the U.S.

Unfortunately, the troubles with the U.S. education system are much deeper than distribution of funding or curriculum weaknesses, although these are both a by-product of the cultural issue that Asimov observes. The troubles lie in the cult of ignorance and anti-intellectualism that has been a long-standing part of American society and which has become increasingly evident and powerful in recent years through the propagandizing and proselytizing of groups like the Tea Party and the religious right.

The fundamental reason that countries in places like East Asia present such a significant challenge to the U.S. politically and economically is not because they have a lot of people or big militaries, or seem to be willing to grow their economic and political might without concern for issues like damage to the environment (China). The problem is that these countries have core cultural values that are more akin to a cult of intelligence and education than a cult of ignorance and anti-intellectualism. In Japan, for example, teachers are held in high esteem and normally viewed as among the most important members of a community. I have never run across the type of suspicion and even disdain for the work of teachers that occurs in the U.S. Teachers in Japan typically are paid significantly more than their peers in the U.S. The profession of teaching is one that is seen as being of central value in Japanese society and those who choose that profession are well compensated in terms of salary, pension, and respect for their knowledge and their efforts on behalf of children.

In addition, we do not see in Japan significant numbers of the types of religious schools that are designed to shield children from knowledge about basic tenets of science and accepted understandings of history—such as evolutionary theory or the religious views of the Founding Fathers, who were largely deists—which are essential to having a fundamental understanding of the world. The reason for this is because in general Japanese value education, value the work of intellectuals, and see a well-educated public with a basic common knowledge in areas of scientific fact, math, history, literature, etc. as being an essential foundation to a successful democracy.

Americans need to recognize that if the cult of ignorance continues, it will become increasingly difficult to compete politically and economically with countries that highly value intelligence and learning. Nowhere is this more problematic in the U.S. than among a growing number of elected officials who are products of that cult of ignorance and who, thus, are not equipped to compete with their international peers. Why is this a problem of national security? Because a population and its leadership need to have the knowledge and intellectual skills necessary to analyze world affairs in an intelligent and sophisticated way and to elect intelligent, capable representatives. The problem is not really with our educational system; it is with our educational culture. Americans need to remember the words of Thomas Jefferson, who wrote to Charles Yancey on January 6, 1816:

“if a nation expects to be ignorant & free, in a state of civilisation, it expects what never was & never will be.”

Wonders never seem to cease when mainstream media, and elements of alternative media espouse the same propaganda. We see this in the coverage of the ‘diseased’ illegal aliens (children) ‘flooding’ the U.S. from Mexico who are in fact descendents of the U.S. before Europeans flooded America bringing diseases that wiped out whole indigenous communities.

In the case of the missing Israeli settlers occupying Palestinian land, mainstream and alternative media automatically assume that Palestinians did kill the Israeli teenagers. Given the hell that Palestinians have been through aided and abetted by their neighbours, it is natural to assume that they did indeed kill the Israeli teenagers; however not everyone behaves in the same way. Israel has made many attempts to ignite the region as a means to achieve its long term goals.

Short memories of Zionist history have bypassed what Zionists are capable of doing to their own, particularly Jews, but how many are reporting that Israeli authorities have confirmed  arrest of a number of Jewish suspects in the death of a Palestinian teenager whose death set off violent protests. Or the fact that cousin of murdered Afula teen Shelly Dadon accused the Israel Police Sunday of purposly hushing up the real motive.  Even speaking on Voice of Israel public radio, Shelly Dadon said:

“We are under a gag order and we still cannot say how many times he stabbed her and what he did to her body afterward,”

believing the murderer  to be an Israeli citizen.

Evidence that came to light after Israel removed its gag order on information regarding the June 12 kidnapping and murder of three Jewish Israeli students suggests that it could have been an Israeli government operation that was intentionally used to punish Hamas and break up the new Palestinian unity government.

Israeli Prime Minister Benjamin Netanyahu had immediately accused Hamas of the kidnapping — without presenting any evidence — and proceeded to “conduct a search” throughout the entirety of the West Bank until the bodies were reportedly found on June 30th.

The “search” entailed arresting and beating up about 600 Hamas members (including legislators) and trashing about 2100 homes; Israeli forces killed at least 7 Palestinians. Israel also heightened its daily air strikes on the Gaza Strip, which has been under the Israeli blockade since Sept. 2006. The Gaza government appealed to the UN for relief, which responded by condemning the kidnappings rather than the massive abuse of the Palestinian population. And then urging “all parties” to show restraint.

On July 1, Israel removed a gag order on information about the kidnapping that revealed shocking facts:

  • The Israeli government had informed members of the press around June 15th that it was aware that the students had been killed (1) but placed a gag order on that information: the government must thus have known where the bodies were.  The brutal “search” was merely cover for punishment of members of Hamas, the democratically-elected party of Palestinians throughout the occupied territories. The Israeli media played along with the pretext for the abuse.
  • The Israeli Defense Forces (IDF) were only called in 8-9 hours after the first call to report the kidnapping. One of the students placed a call to the police emergency hotline 10 minutes after they accepted a hitchhiking ride home, with the message that he was being kidnapped. The statement was followed by what sounded like several gunshots, groans and silence; the call lasted for 49 seconds. Police ignored the call. The teen’s father called police 5 hours later, at 3:30 am, to report his son missing. “Several hours” after that, after an exchange of 54 phone calls, the IDF and Shin Bet finally became involved. (2)
  • Despite possession of all of the evidence of the kidnappings and murders, the Israeli government has offered no evidence that indicates responsibility for the acts. Those with any direct connection to the phone calls or the finding of the bodies have remained unidentified.

Prime Minister Netanyahu has used this event for huge political gain:

  • to create divisions within the new “unity government” of Fatah and Hamas,
  • to physically punish Hamas members and the cause of Palestinian resistance,
  • to get legislation passed through the Knesset to block the return of East Jerusalem to Palestinians (1),
  • to try to foment a third intifada to legitimate further attacks on Palestinians (1) and
  • to whip up such hatred of Palestinians that it has become dangerous for them to be seen on Israeli streets.

The Israeli government is continuing to harass and attack Palestinians until it supposedly finds the killers. The two young Hebron men named as the accused have been missing since June 12th.

The uncovering of this deception should arouse world condemnation.
